Presbyterian Medical Group seeks a Board Eligible / Board Certified Internal Medicine or Geriatric Care physician to join our established practice in Albuquerque, New Mexico. We're all about well-being, starting with yours. We offer a nationally competitive salary with relocation allowance available, CME allowance and fully paid malpractice insurance.

Practice Highlights:

  • Average of 6-8 patients per day
  • Call is in rotation with the group
  • Complete Care team consists of multi-displinary providers and other clniical staff

Responsibilities

Summary:
Provides comprehensive medical services for patients in a program of home-based and clinic care designed to serve the needs of medically fragile patients with complex conditions. Patients (and their family members) will need well-coordinated medical care that addresses their personal goals for well-being and quality of life. Care will include primary care, urgent care and Hospital at Home care, as well as collaboration with nurse case managers, care coordinators, behavioral health specialists and consulting physicians and other healthcare professionals who comprise the patients care team. The Complete Care Medical Director and Clinic Lead physician lead the care teams in providing efficient and compassionate care. Care for patients may extend to locations outside the home and clinic, such community nursing homes and assisted living facilities.

 

Responsibilities:

  • Oversees and is accountable for the overall quality of care and clinical outcomes of patients seen.
  • This care will be delivered by interdisciplinary team members to assure high quality patient care and outcomes.
  • Primary health care provider, emphasizing a focus on patient and family goals of care and quality of life.
  • Manage acute and chronic diseases, including support for any care transitions.
  • Serves as attending for patients admitted to the Hospital at Home program
  • Offers clinical support to APCs on the team providing care to patients
  • Works to assure patient satisfaction with care.
  • Addresses needs around Advance Care Planning for patients.
  • Performs other functions as required.

Other information:
MD/DO Board eligible or certified in Internal Medicine, Family Medicine, Geriatrics, or other applicable board certified practice. State of New Mexico Medical License required.
