Summary:
As a member of the Presbyterian Post Acute Care Program, you will provide comprehensive medical services for patients in programs of home-based and post-acute care designed to serve the needs of medically fragile patients with complex conditions. Patients (and their family members) will need well-coordinated medical care that addresses their personal goals for well-being and quality of life. Care is given in collaboration with nurse case managers, care coordinators, behavioral health specialists and consulting physicians and other healthcare professionals who comprise the patients care team. The Program Medical Director leads the care teams in providing efficient and compassionate care. Care for patients may extend to locations outside the home and clinic, such as community nursing homes, skilled nursing facilities and assisted living facilities.

MD/DO Board eligible or certified in Internal Medicine, Family Medicine, Geriatrics, or other applicable board certified practice. State of New Mexico Medical License required.
