Presbyterian Medical Group seeks an advanced practice clinician to join our established Hospital at Home practice in Albuquerque, New Mexico. We're all about well-being, starting with yours. Responsibilities

Summary:
Under the supervision of Complete Care physicians, provides Primary Care, Urgent Care and Hospital at Home care to patients seen in the program. Complete Care serves patients have been referred due to their medical fragility and complicated conditions. Patients and family members will need well-coordinated medical care concordant with their personal goals for care. Work will involve direct medical management as well as collaborating with the physicians, pharmacists, nurse case managers, care coordinators, behavioral health specialists and other consulting physicians and other healthcare professionals who comprise the patients care team. Overseeing any care transitions to ensure high quality care will be a component of the work. Patients may be seen in many care settings, including home, clinic, assisted living and nursing home.

 

Responsibilities:

  • Primary health care provider, emphasizing a focus on patient and family goals of care and quality of life.
  • Manage acute and chronic diseases, including support for any care transitions.
  • May admit and round on patients admitted to Hospital at Home, with the supervision of a Complete Care physician
  • Work with interdisciplinary team to ensure high quality patient care.
  • Order, conduct and interpret appropriate diagnostic x-ray and lab tests.
  • Prescribe treatments medications and non-pharmacologic therapies.
  • Involved in teaching and counseling individuals, families and groups.
  • Work autonomously as well as collaborate with other health care providers as needed.
  • Serve as health care resource, interdisciplinary consultant and patient advocate.
  • Performs other functions as required.

Other information:
Masters degree in Nursing, New Mexico Nurse Practitioner license, federal and state DEA license and ANCC certification required. One to three years experience in an inpatient or outpatient setting in family practice, internal medicine, or geriatrics.
