Presbyterian Medical Group seeks a Board Certified Pharmacist Clinician to join our established practice in Santa Fe, New Mexico. We're all about well-being, starting with yours. Practice Highlights:

  • 12-14 patients per day
  • Patient Centered Medical Home Support Team consisting of Physician, Behavioral Health Clinician, Nurse Practitioner, RN, Medical Assistant
  • 24 hours per week of patient care

 

Responsibilities

Summary:
Provides chronic disease state management services and comprehensive pharmaceutical care for patients in the clinical setting and practices under PHS approved clinical scope of practice.

 

Responsibilities:
* Provide comprehensive chronic disease state management in collaboration with primary care providers and/or specialists. Incorporates the use of physical assessment skills, prescriptive authority, and therapeutic drug monitoring as appropriate.
* Perform and document Medication Therapy Management.
* Provide clinical consultation and drug information services as needed via published evidence-based literature and national clinical guidelines.
* Development and review of collaborative practice agreements and clinical pathways
* Provide clinical oversight and consultation for Anticoagulation Clinics
* Mentor and precept Pharmacy Practice Residents in accordance with ASHP accreditation guidelines and UNM College of Pharmacy students.
* Required to participate in peer review and clinical chart audits as needed
* Required to participate in onboarding of newly hired PhCs as needed

 

Credentials Required:
Bachelor of Pharmacy degree required. Doctor of Pharmacy degree preferred. Active New Mexico Board of Pharmacy License (RPh) required. New Mexico Pharmacist Clinician License (PhC) with prescriptive authority within 9 months of employment required. Board Certification granted by Board of Pharmaceutical Specialties required. Minimum 3-5 years of experience in an approved clinical setting or completion of an ASHP accredited Pharmacy Residency program preferred.
